When someone says “I miss you” and you don’t return the sentiment, how do you reply without being hurtful and without lying?

Smile and nod as if you understand. Depending on the person, “I know”, or some other kind of acknowledgment, like “Yea, we haven’t seen each other in a while.”

If you’re looking for them to understand that you don’t feel the same way, saying something like “I understand…” bluntly may do it – it’s all in the tone you project what you’re saying.
