most COLLEGES do.

SOME universities do.

HARVARD ( and yale, cambrisge…. etc.) May, but unlikely.

My college doesn’t. I just had my orientation a few weeks ago and for English they require a 4 or a 5 but for subjects like Science and Math they usually accept a 3. It depends on the school, but all though my school is a university, it’s not like a super difficult one, so if you’re aiming for a pretty good school I’d say it’s likely that they want at least a 4.

Yes, it depends on where you are going though.
