Take her out to a nice restaurant and then out to get her nails done,
You may not enjoy the latter but remember that
this day is about her.
That or get her a certificate to a spa in the area(:

Answer by capricorngirlYou haven’t indicated a budget so it is a little difficult. But my daughters bought me a pandora bracelet for my birthday and have started it with charms that represent family and happiness. I just love it and they always know what to buy me for my birthday now, another charm. Good Luck and hope your mum has a great day.Answer by maxin_96
You can schedule a family portrait and have it framed. I’m sure she would love that, having an update portrait of her family. Even if you have a friend come in and take the picture for you and blow it up and frame it she would love it!
Take her on a dinner cruise.
Take her to show, if there’s one she’s been wanting to see, dinner and a show.
A day at the spa, just the two of you, and then out to lunch afterwards.
Is there somewhere that she’s been wanting to go to, like even for a weekend? You can take her away or to an art gallery, musuem, etc?
Nice restuarant for lunch or dinner.
New tv, being the tvs are all changing in 2009, you can get her a new flat screen.
Computer, if she already doesn’t have one.
How about a road trip? Do you have family members that she’s been wanting to see that you can surprise her and take her on a road trip to go see them?
Weekend getaway? Somewhere that’s not too far, but not close by either that you can go to.
How about a Casino getaway? If she’s into it take her to the Casino for the day or overnight.
Is there something that she would like, but you know she won’t buy for herself? You can do that.
What about the family renting out a cottage or hotel suite and celebrating her birthday at a weekend getaway at the beach or mountains and have everyone chip in to pay for it, and she’ll have everyone there to celebrate it.
Hot air balloon ride? You can look into places around you that do it, but she would have to be up around 5am to get there and it all depends on the wind conditions on whether or not you actually go up that day or not.
Picnic in the park? You can pack a lunch and find a nice area and take her out for lunch in the park if you have one nearby.
Does she have a favorite restuarant or a restuarant that she’s always wanted to go to or try? You can take her there for lunch or dinner.
What about making a day of it, start off with breakfast, take her out, go for breakfast, and then decide what’s next. It could be shopping, the zoo, the spa, whatever, then lunch, and then more in the afternoon, and then dinner. You can even rent a limo to drive you around for the day so the two of you can really enjoy each other’s company and not worry about the driving and parking and traffic, etc.
I hope some of these ideas help and that your mom has a very Happy Birthday!
